IMPORTANT WARNING: Please do not cancel any health insurance you currently have until you have received,
reviewed and accepted a new health insurance policy. Unlike many other types of insurance, such as auto or homeowners
insurance, your Agent is not authorized to bind (begin) coverage. Health insurance will not go into effect until
your application is approved by an underwriter and their offer of coverage is accepted by you. Their offer of coverage
is also contingent upon no changes in your health between the time of application and the time you accept their
offer of coverage. Their offer of insurance to you might also include limitations or exclusions of any pre-existing
health conditions you may have. Therefore it is very important for you to wait until you receive and review any
offers of insurance from your new company before cancelling any insurance you currently have. You might not be
able to get that coverage re-started if you cannot secure a new policy or cannot do so on terms acceptable to you.

favorite doctor(s) &/or hospitals are in the network(s) you are interested in, it is always best to check with
your doctor to see whether or not (s)he is currently in the network and intends to remain there. Participants in
the various networks change over time. Your doctor may not be currently listed in the network but is in the process
of being added, or they may be listed but are contemplating leaving.
